Plan MembersWith a secure member account, you have benefit summary information for you and your dependents at your fingertips. You can view a benefit summary, certificate of coverage, pending and paid claims, pretreatment estimates and the status of plan maximums and deductibles.

Your secure account lets you quickly verify patient benefits, view dental and/or orthodontic claims, confirm payment amounts and check dates for processed claims.

We currently serve the vision care needs of approximately 159 million members through 4,000 clients and access to thousands of private practice and retail-affiliated network providers.
EyeMed offers a variety of plan options to help our clients provide the vision care best suited to their employees' needs. A combination of copayments, allowances and discounts helps us create a flexible plan and complements existing medical and ancillary coverage.

Landmark is dedicated to serving the needs of its health plan, TPA and employer group clients.
Since 1985, seamlessly integrated physical medicine administration, care management, analytics, and patient focused programs have consistently delivered high quality clinical outcomes at a reasonable cost to clients.
Landmark Healthplan of California provides insured chiropractic, acupuncture and combined chiropractic/acupuncture benefit plans for more than 150,000 fully-insured and self-funded members in California.
- Network-based benefit plans offer flexible copay and visit limit options that are available to health insurance carriers and employer groups of 2 or more employees.
- Plan designs meet a wide range of employers' health and wellness objectives - Standard Plan or Expanded Plan.
- Select from network, medical management and claims processing services for self-funded employer plans.
- Members have direct access to any chiropractor or acupuncturist on Landmark's fully credentialed network and may change practitioners at any time. Members call their selected practitioner, schedule their appointment and pay the appropriate copay when they present for services.
- Proposed treatment is peer reviewed for medical necessity according to Landmark's evidence based guidelines to ensure, safe, quality, affordable care.
Who is Madison National Life Insurance Company?
Madison National Life (MNL), rated A-(Excellent) by A.M. Best Company, is one of three insurance companies that are part of the IHC Group. The IHC Group is an organization of insurance carriers, managing general underwriters, third-party administrators and marketing affiliates that has been providing life, health, disability, dental, vision and medical stop-loss insurance solutions to groups and individuals for more than 25 years.*
The MNL is the principle carrier for IHC Group's dental and vision products**. The MNL dental products are offered for individuals, small and large groups, associations, fully-insured and self-insured employers. The dental products are available on an indemnity as well as PPO basis.
